Sleep Apnea Treatment in Los Angeles: A Holistic Dentist’s Approach to Airway Health

SLEEP APNEA AND AIRWAY DENTISTRY · LOS ANGELES
Because how you breathe at night shapes how you live during the day.
Sleep apnea is one of the most underdiagnosed conditions in the United States, and the mouth is where its structural causes begin. Narrow jaws, low tongue posture, and restricted airways are anatomical patterns that holistic and biological dentists are trained to recognize. At Rite Dentist in Los Angeles, we screen for airway dysfunction and offer oral appliance therapy as a first-line treatment for mild to moderate obstructive sleep apnea, providing a comfortable alternative to CPAP for many patients.

Why a Dentist for Sleep Apnea?

Sleep apnea is a breathing disorder. But the anatomy that causes it is entirely within a dentist’s scope of expertise.

Obstructive sleep apnea occurs when the soft tissues at the back of the throat collapse during sleep, blocking the airway. The structural factors that make this more likely include a narrow dental arch, a posteriorly positioned lower jaw, a low-lying soft palate, and poor tongue posture. All of these involve the bones and soft tissues that dentists spend their careers studying.

Biological and airway-focused dentists screen for these structural risk factors at every exam. We look at palate width, jaw position, tongue-tie involvement, and signs of nighttime grinding (bruxism), which often signals the brain trying to reopen a restricted airway. When we see a pattern that suggests sleep-disordered breathing, we discuss it with the patient and coordinate with sleep medicine physicians when appropriate.

Oral Appliance Therapy: A CPAP Alternative for Mild to Moderate Sleep Apnea

CPAP (continuous positive airway pressure) is the standard medical treatment for sleep apnea. It is highly effective when used, but compliance rates are poor. Research published in the Journal of Clinical Sleep Medicine puts CPAP adherence at roughly 50%, meaning half of patients prescribed a CPAP machine do not use it consistently.

Oral appliance therapy provides a well-studied alternative for patients with mild to moderate obstructive sleep apnea. The appliance is a custom-fitted device that gently advances the lower jaw during sleep, keeping the airway open. It is quiet, portable, requires no electricity, and most patients adapt to it far more readily than to CPAP.

The American Academy of Sleep Medicine recommends oral appliance therapy as a first-line treatment for mild to moderate sleep apnea and as an alternative to CPAP for patients who prefer it or cannot tolerate the machine.

At Rite Dentist, we fabricate custom oral appliances using digital impressions and biocompatible materials. We work collaboratively with your sleep physician to ensure the appliance is calibrated to your anatomy and that treatment outcomes are measured properly.

Signs you may have sleep apnea
Loud snoring, waking with headaches, daytime fatigue despite a full night’s sleep, waking frequently, poor concentration, teeth grinding, and a partner noticing that you stop breathing during sleep are the most common indicators. Many people with sleep apnea have no idea until a bed partner observes an episode.
The systemic toll of untreated sleep apnea
Untreated sleep apnea has been linked to hypertension, heart disease, type 2 diabetes, stroke, and depression. Each apnea event triggers a stress response that elevates cortisol and blood pressure. Over years, these nightly interruptions compound into measurable cardiovascular and metabolic damage.

Frequently Asked Questions: Sleep Apnea Treatment in Los Angeles

Can a dentist diagnose sleep apnea?
A dentist can screen for the structural risk factors associated with sleep apnea and identify clinical signs that suggest sleep-disordered breathing. We cannot diagnose sleep apnea ourselves — that requires a sleep study (polysomnography or an at-home sleep test ordered by a physician). What we can do is recognize the warning signs, refer you for proper testing, and provide oral appliance therapy once a diagnosis is confirmed by your doctor.
How does a dental oral appliance help with sleep apnea?
A mandibular advancement device (MAD) works by gently holding the lower jaw in a slightly forward position during sleep. This prevents the tongue and soft tissues at the back of the throat from collapsing into the airway. The result is a clearer breathing pathway throughout the night. The appliance is custom-fitted to your bite and adjusted over time to achieve the optimal jaw position for airway opening.
Is oral appliance therapy as effective as CPAP?
For mild to moderate obstructive sleep apnea, oral appliances are clinically comparable to CPAP in effectiveness. For severe sleep apnea, CPAP tends to be more effective at reducing the apnea-hypopnea index. However, because CPAP compliance is so poor, many patients see better real-world outcomes with an oral appliance they actually wear every night. We discuss this tradeoff with patients honestly and coordinate with their sleep physician on the appropriate choice.
Do I need a sleep study before getting an oral appliance?
Yes. We do not fabricate oral appliances without a confirmed sleep apnea diagnosis. Snoring alone does not indicate sleep apnea, and treating the wrong condition wastes time and money. If you have not had a sleep study and are experiencing symptoms, we will refer you to a sleep medicine specialist and coordinate your care once a diagnosis is in hand.
What does an oral appliance fitting involve?
We take digital impressions of your teeth and record the relationship of your upper and lower jaws. This data goes to a dental laboratory where your appliance is fabricated. At your fitting appointment, we check the fit, confirm the jaw position, and show you how to insert, remove, and clean the device. Follow-up appointments allow us to fine-tune the jaw advancement until your symptoms resolve.
Are there holistic lifestyle approaches that help sleep apnea?
Yes. Weight management, sleeping on your side rather than your back, reducing alcohol and sedatives before bed, addressing nasal congestion, and myofunctional therapy exercises to strengthen the tongue and throat muscles can all reduce sleep apnea severity. We discuss these approaches alongside appliance therapy because the most effective outcomes typically come from combining structural treatment with lifestyle support.

Sleep Better. Live Better.

If you snore, grind your teeth, or wake exhausted, your airway may be the problem. Book an airway screening at Rite Dentist in Los Angeles and find out if oral appliance therapy is right for you.
