Titanium has been the standard implant material for decades, with a well-established safety profile and osseointegration success rates above 95%. Zirconia, a ceramic form of zirconium dioxide, has been used in dentistry since the 1990s and in implant form since the early 2000s. Clinical studies, including a 2016 systematic review in the International Journal of Oral and Maxillofacial Implants, show survival rates for zirconia implants comparable to titanium at five-year follow-up.
The practical differences matter most for certain patients. Zirconia is white, so there is no dark metallic line visible at the gumline if recession occurs. It contains no metal, making it the appropriate choice for patients with known titanium sensitivities or those who prefer a fully metal-free mouth. Zirconia also has lower plaque affinity than titanium, meaning less bacterial accumulation on the implant surface.
The primary trade-off is that zirconia implants are typically one-piece (the implant and abutment are fused), which limits some positioning flexibility compared to two-piece titanium systems. Not every patient or implant site is an ideal candidate. We assess this carefully at consultation.
