Most people think of it as a gum and tooth problem, but a growing body of research shows it functions more like a chronic systemic inflammatory condition that happens to originate in the mouth. Diabetes: The relationship between gum disease and diabetes runs in both directions. Uncontrolled blood sugar creates conditions that accelerate periodontal breakdown, and active gum disease makes blood sugar harder to control. A 2013 Cochrane review found that treating periodontal disease was associated with a meaningful reduction in HbA1c levels in diabetic patients.
Cognitive decline: Porphyromonas gingivalis, one of the primary bacteria in periodontal disease, has been detected in the brains of Alzheimer’s patients. A landmark 2019 study in Science Advances found gingipains (enzymes produced by P. gingivalis) in brain tissue and showed they could induce Alzheimer’s-like pathology in animal models. The research is ongoing, but the implications have prompted significant scientific attention.
Pregnancy outcomes: Multiple studies have linked periodontal disease in pregnant women to increased risk of preterm birth and low birth weight. The proposed mechanism is the elevation of prostaglandins and cytokines triggered by periodontal pathogens, which can stimulate uterine contractions. The American Academy of Periodontology recommends that pregnant women receive a periodontal evaluation during pregnancy.
Respiratory disease: Bacteria from infected gum tissue can be aspirated into the lungs, contributing to pneumonia and exacerbating chronic obstructive pulmonary disease (COPD). Patients in hospital settings with poor oral hygiene have significantly higher rates of respiratory complications.
