We Exhaust Every Option Before Removing a Tooth
Losing a tooth is never our first instinct — it’s always our last resort. We believe in doing everything possible to save a natural tooth before considering extraction, because nothing replaces what you were born with. When we do recommend an extraction, it is because we have genuinely exhausted the alternatives and determined that removal is the best path forward for your overall oral health.
The situations that may lead us to recommend extraction include: severe decay that has destroyed too much of the tooth structure to support a crown or other restoration; advanced periodontal disease where the supporting bone has been lost to the point that the tooth can no longer be stabilized; an impacted wisdom tooth causing pain, infection, or damage to neighboring teeth; a tooth fractured at or below the gumline where there is not enough remaining structure to restore; and in some cases, overcrowding that must be addressed before orthodontic treatment can proceed.
There are two types of extractions. A simple extraction is performed when the tooth is fully erupted above the gumline — we loosen the tooth with an elevator instrument and remove it with forceps. A surgical extraction is needed when a tooth is broken at the gumline, impacted, or has curved roots that don’t allow for straightforward removal. Both procedures are performed under local anesthesia, and nitrous oxide sedation is available if you’re feeling anxious.
Before you leave our office, we’ll go over detailed aftercare instructions and discuss your tooth replacement options. Leaving a gap after extraction can lead to shifting of neighboring teeth, bone loss at the extraction site, and bite changes over time. We’ll talk through options like implants, bridges, and partial dentures so you understand your choices and can make a decision that’s right for you.

What patients ask about tooth extractions
Is tooth extraction painful?
The area will be thoroughly numbed with local anesthesia before we begin, so you should feel pressure and movement but not pain during the extraction itself. Afterward, as the anesthesia wears off, it's normal to experience soreness for a few days. We'll prescribe or recommend appropriate pain management and give you detailed aftercare instructions to keep you comfortable during healing.
When do I need a tooth extracted vs. a root canal?
A root canal is an attempt to save the tooth by removing the infected pulp tissue while leaving the natural crown intact. If the tooth structure above the gumline is still sufficient to support a restoration, a root canal and crown is usually the preferred option. Extraction becomes necessary when the tooth is too damaged, too infected to save, or the infection has spread in a way that makes retention unsafe. We'll always give you our honest assessment of which path makes the most sense.
How long is the recovery after an extraction?
Most patients feel comfortable returning to normal activities within two to three days for a simple extraction, and four to seven days for a surgical extraction. Complete socket healing takes several weeks. During this time, you'll want to avoid smoking, drinking through straws, and eating hard or crunchy foods near the extraction site. We'll provide a full list of aftercare instructions and follow up with you after your procedure.
Will I need to replace the extracted tooth?
For most teeth — yes, replacement is strongly recommended. When a tooth is missing, the surrounding teeth gradually drift into the gap, the bone beneath the extraction site begins to resorb, and your bite can shift in ways that affect chewing efficiency and jaw health. Dental implants, fixed bridges, and removable partial dentures are all options we can discuss. The exception is wisdom teeth — these are not typically replaced because they serve no functional purpose.
Can I be sedated for a tooth extraction?
Yes. We offer nitrous oxide (laughing gas) sedation for patients who feel anxious about extractions. Nitrous oxide takes effect quickly, creates a feeling of relaxation and detachment, and wears off completely within minutes after we stop administering it — so you can drive yourself home. If you have significant anxiety about the procedure, please let us know when you book and we'll make sure sedation is ready for you.
