Root canal therapy (endodontic treatment) removes infected or inflamed pulp tissue from inside the tooth, seals the root canals, and crowns the remaining structure. When a tooth is severely infected with irreversible pulpitis (inflammation that cannot heal on its own), root canal treatment is often the correct choice — and significantly better than extraction when the goal is saving the tooth.

However, not every tooth that generates pain or sensitivity requires root canal treatment. Early infections, reversible pulpitis, deep cavities approaching the pulp, and some cracked teeth can sometimes be treated conservatively — with ozone therapy, indirect pulp capping, or MTA (mineral trioxide aggregate) pulp capping — that allow the pulp to heal and recover without removal.

When Is a Root Canal Necessary?

Root canal treatment is appropriate when:

The dental pulp is irreversibly inflamed or necrotic (dead)

A dental abscess (pus-filled infection) is present

The infection has spread beyond the tooth to surrounding bone

Pain is severe, spontaneous, and not responding to conservative treatment

Significant structural damage leaves too little healthy tissue for other options

Conservative Alternatives (When Applicable)

When the pulp is still vital (alive) and the infection or inflammation is caught early enough, conservative options may allow the tooth to heal:

Ozone therapy: Ozone gas or ozone water is applied directly to the affected area, eliminating the bacterial cause of pulpal inflammation. When a deep cavity has not yet fully penetrated the pulp, ozone-assisted treatment combined with a biocompatible liner can allow the pulp to recover. Research shows ozone eliminates 99 percent of caries-causing bacteria on contact (Baysan et al., 2001, European Journal of Dentistry).

Indirect pulp capping: When decay is removed to just short of the pulp, a biocompatible material (calcium hydroxide or MTA) is placed over the remaining thin dentin layer, stimulating the pulp to form reparative dentin and seal itself. Success rates of 80 to 95 percent have been reported in the literature for properly selected cases.

Direct pulp capping: When a small pulp exposure occurs (often during cavity preparation), a biocompatible capping material is placed directly on the exposed pulp tissue. Success depends on the size of the exposure, absence of contamination, and patient age. Cases are carefully selected.

Re-evaluation: Some initial tooth pain resolves without intervention once a stressful stimulus is removed. Dr. Babrood may recommend a period of monitoring with specific home care before committing to irreversible treatment.

Are there alternatives to root canals?
In some cases, yes. When the dental pulp is still vital and the infection is caught early, ozone therapy, indirect pulp capping, or direct pulp capping may allow the pulp to recover without removal. However, if the pulp is already irreversibly inflamed or necrotic, root canal treatment or extraction are the only options. The key distinction is whether the pulp is reversibly or irreversibly affected — which requires clinical and radiographic evaluation.
What is ozone therapy for tooth infections?

How do I know if I need a root canal or just a filling?
Pain characteristics help differentiate: spontaneous pain, pain that lingers long after a stimulus is removed, or pain that wakes you at night suggests irreversible pulpitis (root canal likely needed). Brief, sharp pain that resolves quickly when a cold or pressure stimulus is removed suggests reversible pulpitis (may heal conservatively). A dentist must take X-rays, test pulp vitality, and evaluate clinically — never diagnose based on a description alone.
What happens if I don't get a root canal when I need one?
An irreversibly infected tooth that goes untreated will develop a dental abscess — a bacterial infection that spreads into the surrounding bone and soft tissue. Left untreated, dental abscesses can spread to the jaw, neck, or other structures. Serious complications, while rare, include Ludwig’s angina (infection of the floor of the mouth) and mediastinitis (chest infection). A tooth that needs root canal treatment should not be ignored; the question is whether to treat or extract, not whether to act.
