Understanding TMJ disorder
Your Jaw Pain Has a Source — and a Solution
The temporomandibular joint — your TMJ — is the hinge that connects your lower jaw to your skull, just in front of each ear. It’s one of the most complex joints in the body, responsible for every bite, chew, and word you speak. When something disrupts this joint or the muscles surrounding it, the result can range from mild discomfort to chronic, debilitating pain that spreads far beyond the jaw itself.
TMJ disorder (TMD) is an umbrella term for a group of conditions affecting the joint and the surrounding musculature. Symptoms vary widely but commonly include: a clicking, popping, or grating sensation when opening or closing the mouth; pain or tenderness in the jaw, face, or temples; difficulty opening the mouth fully; pain while chewing; earaches or ringing in the ears with no apparent ear problem; morning headaches; and tension radiating into the neck and shoulders. Many patients are surprised to learn that their chronic headaches or ear pain has a dental origin.
Common causes include bruxism (nighttime teeth grinding and clenching), bite misalignment that places uneven forces on the joint, stress-related muscle tension, prior trauma to the jaw, and arthritis within the joint itself. In many cases, multiple factors are at play simultaneously. Identifying the underlying cause is essential to effective treatment — which is why we take a thorough history and conduct a careful clinical evaluation before recommending anything.
Treatment options at our office focus on conservative, reversible approaches first. Custom occlusal splints (night guards) protect the joint during sleep and interrupt the grinding cycle. Bite adjustment may be performed to reduce uneven forces. We also provide guidance on jaw exercises, stress management techniques, and dietary modifications that reduce joint strain. Surgery is rarely necessary and is never something we rush toward — the vast majority of TMJ patients achieve meaningful relief with conservative care.
Common Questions
What patients ask about TMJ treatment
What does TMJ disorder feel like?
TMJ disorder most commonly feels like jaw pain or soreness, especially when chewing or first waking up. You may notice clicking or popping sounds when you open your mouth, difficulty opening wide, or a sensation that your jaw "catches." Many patients also experience headaches, ear pain, or tension in the neck that they don't associate with their jaw until a dental evaluation reveals the connection.
How do I know if I have TMJ or just jaw soreness?
Occasional jaw soreness after a long meal or vigorous chewing is normal. TMJ disorder is characterized by persistent or recurring symptoms — pain that returns regularly, clicking that accompanies every opening, headaches that occur most mornings, or limited jaw range of motion that doesn't resolve. If your symptoms have lasted more than a few weeks or are affecting your daily life, a dental evaluation is a good next step.
Can a dentist treat TMJ disorder?
Yes. Dentists are uniquely positioned to diagnose and treat many forms of TMJ disorder because the causes are often occlusal (bite-related) or the result of bruxism. We evaluate your bite, assess joint function, review your symptoms, and create a treatment plan. For complex cases involving significant joint damage, we may co-manage care with an oral surgeon or physical therapist, but most cases respond well to dental treatment alone.
Will I need surgery for TMJ?
Surgery is rarely necessary for TMJ disorder. The vast majority of patients — even those with significant symptoms — achieve meaningful relief through conservative, non-surgical treatments like night guards, bite adjustments, exercises, and lifestyle changes. We always start with the least invasive approach and only refer for surgical consultation in cases where conservative treatment has failed after a thorough trial.
Does a night guard help with TMJ?
For many patients, yes — a custom occlusal splint is one of the most effective first-line treatments for TMJ disorder caused by bruxism or bite imbalance. The splint prevents the teeth from grinding against each other during sleep, reduces muscle tension, and takes stress off the joint. It doesn't cure the underlying cause, but it relieves symptoms significantly while we work on other contributing factors. Not all TMJ cases are the same, so we'll assess whether a night guard is the right tool for your specific situation.
